Where to apply may be solved by implementing two types of analysis; Basic 'Failure Analysis' and "Root Cause Analysis". Let the 'Failure Analysis' be done more or less on the fly, with KISS practices. Once a month review the 'Failure Analysis' and apply RCFA only to those with popular belief that a 'Root Cause' wasn't found or is needed.
