People with less knowledge about RCFA proccess, but normally usedto be listen to, tend to to derail or try to pull the proccess to a certain direction. If a person holds a valuable information with him and you don't want to loose him/her in the future meetings,after the session go and thank them for their valuable input in the proccess. You will see a person with different attitude in the following meetings.

Immediate cause identification is very effective, however, it is very common to see that not enough "effort" is placed on identification of latent organizational or management system causes. It seems to me that lack of understanding about what a latent cause is might be the problem.

I think that with the proper training and education, people will learn that RCFA is not about finger pointing or punishing the person who made the mistake but understanding why the mistake occured. If we can clearly educate people that RCFA is not about punishing people then I think people will buy to this strategy
