I believe that the people assigned to do a RCA has always the best intention to do it, but I have seen so many cases where the same RCA is a failure event(root cause or right corrective action is not addressed) because the person facilitating or leading the RCA doesn't have the skills to do it.

I also rate this high since everyone will require education and training prior to any RCFA undertaking. RCFA is a different ballgame from the same things that they are doing and that's firefighting. It requires a different mindset
